Jake Clark-Bellenfant is the Medical Director and Provider at Symmetry Solutions. He oversees and provides psychiatric evaluations and follow-up care for those receiving Ketamine Assisted Psychotherapy and medication management. He received his Bachelor of Science in Nursing from the University of Utah and graduated from the Psychiatric Mental Health Nurse Practitioner program at Vanderbilt University.

Jake has 12 years of professional experience in Mental Health and has worked with both youth and adults with a variety of mental health diagnoses in residential, inpatient, correctional, and outpatient settings as a psych tech, Registered Nurse, and Psychiatric-Mental Health Nurse Practitioner.

Jake is currently licensed in Utah, Idaho, Colorado and Nevada.

Jake’s rates are as follows:

$300 per 60-minute initial Adult (age 18+) Medication Management Assessment
$400 per 60-minute initial Adolescent (age 14 to 17) Medication Management Assessment
$150 per 30-minute follow-up Medication Management Session
$350 per 60-minute initial Pre Ketamine Psychiatric Assessment
$175 per 30-minute post Ketamine Follow-up Session
